At Mojab Clinic, we strive to provide a comfortable and respectful environment for all our patients and staff. To ensure that everyone has a pleasant experience, we have implemented the following noise policy:
Appointment Notification
Our medical office will notify you and your family physician about your appointment with Dr. Hamid Mojab
We have informed you about the appointment date and time.
Office of Dr. Hamid Mojab, Endocrinology, Diabetes and Metabolic CentreAs per her new government mandates effective December 01/2022, all appointments will now be contacted in person only. Exceptions will only be made under very special circumstances and at the discretion of their physicians.
- Please inform your patient to bring the list of medications and their new health card, we can not accept the expired health card.
- Please inform your patient that there will be a $150 charge for no-show or less than 48 hours cancellation notice.
- Due to the nature of the care offered at our clinic, we cannot accurately predict the exact time the doctor will be seeing you, please always allow at least 2 hours for all appointments as delay may be expected
- Patients must contact the clinic to confirm their appointment. Failure to do so will result in an automatic cancellation
Appointments, tests and procedures may be canceled or rescheduled as emergencies can occur - Comment for referring professional:
Please note while patients are awaiting elective consultation, our office cannot accept responsibility for their care until the patient has actually been seen. - All patients must present a VALID HEALTH CARD at each visit.
- All patients having in-person consultations required the use of a facemask as COVID protocols remain in effect at our clinic.
- Please note while patients are awaiting an elective consultation by our physician, our centers cannot accept responsibility for the care of the referred patient until they have been seen in consultation. As their referring professional, you remain responsible for the care of your patient until the patient has been seen and consulted at our center.
- Please make sure you provide current contact information for your patient and include an email address and a pharmacy number
- Kindly fax all labs, ultrasound reports, a list of all current medication, and any investigation related to the visit. To the appointment
- Please inform your patient about no show, not attended appointment or cancellation of appointment in less than 48 hours fee of $ 150 with our office as soon as possible
- Patients must bring a valid and not expired OHIP card or proof of insurance coverage to all appointments, We can not accept or process a non Valid or expired Health card

Appointment Policy
To ensure the best possible service, we have established the following appointment policy:
Punctuality
Please arrive at least 10-15 minutes before your scheduled appointment time to complete any necessary paperwork and to ensure your appointment starts on time.
Late Arrivals
- If you arrive more than 15 minutes late for your appointment, we may need to reschedule your visit to avoid inconveniencing other patients.
- Repeated late arrivals may result in a late fee or require pre-payment for future appointments.
Missed Appointments:
- If you are unable to attend your scheduled appointment, please notify us at least 24 hours in advance.
- Failure to provide adequate notice will result in a no-show fee.
No-Show Fee
- A no-show fee of $65 for follow-up appointments and $150 for initial consultations will be charged for missed appointments without prior notice, in accordance with OntarioMedical Association guidelines.
- This fee must be paid before scheduling your next appointment.
Payment Policy:
- No-show fees and any outstanding balances must be settled promptly.
- Patients who do not pay their no-show fees may be required to pre-pay for future appointments or may have their ability to schedule future appointments limited.
Communication
• We understand that unforeseen circumstances can occur. If you experience an emergency or have any concerns about your appointment, please contact us as soon as possible to discuss your situation.
Noise Policy

Quiet Zones
- The clinic waiting area and treatment rooms are designated as Quiet Zones. Please keep noise to a minimum in these areas.
Phone Use
- Patients are kindly asked to set their mobile phones to silent or vibrate mode while inside the clinic.
- Phone calls, video calls, and playing music are not permitted in the clinic. If you need to take a call, please step outside the clinic.
Use Of Headphones:
- If you wish to watch videos or listen to music while waiting, please use headphones to avoid disturbing others.
Respect For Others
- Please be mindful of the volume of your conversations to ensure that other patients can enjoy a peaceful environment
